Every client is required to complete an Intake Packet prior to beginning services.

If you are interested in using private medical insurance to  cover the costs of therapy, please see ourInsurance Information Form and submit along with your intake packet.

Please download the forms, fill out completely and submit  via mail or fax before your appointment. This will help to ensure a timely  first visit.
